What the STEP 1 extension reported
In the STEP 1 extension of semaglutide 2.4 mg, participants had lost 17.3% of body weight by week 68. A year after stopping, they had regained 11.6 percentage points of that loss, about two thirds.
That study is one reason many people ask about maintenance planning before they stop. It does not tell any one person what will happen, but it does show that regain can occur after treatment ends.
Another trial, SURMOUNT-4, looked at tirzepatide. Adults who completed 36 weeks of tirzepatide and then switched to placebo gained 14.0% of body weight from week 36 to week 88, while those who continued tirzepatide lost a further 5.5%.
