Weight regain after stopping Wegovy: STEP 1 extension

Stopping a GLP-1 can be followed by weight regain, and that is what the STEP 1 extension measured after semaglutide was stopped. This page explains what the study reported, how to read it carefully, and what people often track during this period with their prescriber or care team.

Talk with your prescriber or care team before you change, lower, or stop any medication. Ozempic, Wegovy, Mounjaro, and Zepbound are named only to describe what readers search for.

What the STEP 1 extension reported

In the STEP 1 extension of semaglutide 2.4 mg, participants had lost 17.3% of body weight by week 68. A year after stopping, they had regained 11.6 percentage points of that loss, which was about two thirds of it.

This is one reason people search for stop wegovy weight regain. The study shows what happened in that trial after treatment ended, but it does not tell any one person what will happen for them.

Another trial, SURMOUNT-4, looked at tirzepatide. Adults who completed 36 weeks of tirzepatide and then switched to placebo gained 14.0% of body weight from week 36 to week 88, while those who continued tirzepatide lost a further 5.5%.

How to read the numbers carefully

These studies are useful because they show that weight change after stopping a GLP-1 can be substantial. They also show that results can differ depending on the medication and the trial design.

Still, a trial result is not a prediction. It does not include every reason a person may gain, maintain, or lose weight over time. Real life can involve changes in appetite, routine, sleep, activity, stress, and follow-up care. Only your prescriber or care team can guide any medication decisions.

If thinking about food feels hard, talk with a clinician. That support can help you look at the whole picture without blame or pressure.

What you can track

Some people find it helpful to track a few simple signals after a GLP-1 is stopped or changed, especially if they are trying to notice patterns rather than chase a number.

The app will store data on the device, without an account, and it will include export and delete everything options. It will also have a free tier and an optional Pro subscription. If you want a simple way to organize those records, you can review the weight trend and regain band feature or try the regain band calculator.
